LIMITED DENTAL WARRANTY

Investment in optimal health is by far your wisest long-term decision. Just Smiles is very proud of the general dentistry that we provide for you and your family and we strive to exceed your expectations. Our goal is not to just correct any dental problems you may have, but to show you how to prevent dental disease in the future to save you time and unnecessary expense.

The long term success of the dental treatment we provide depends upon your continuing home care, regular professional cleanings, exams and, if necessary, the use of adjunct treatments. We recommend at the minimum that regular visits be 6 months apart. So many factors influence the longevity of dental restorations that our best estimates are simply that: estimates! However, it is our pleasure to offer you what we believe to be a unique value:

DENTAL SEALANTS

Sealants are plastic coatings which are placed on the chewing surfaces of adult teeth to help protect against decay in the pits and grooves. These are the most common areas to get dental cavities in children. Floss and fluoride will help prevent decay between the teeth. We will repair or replace sealants for a period of 5 years after placement, only if you visit us every 6 months. If decay is present on the chewing surface, the cost of the sealant will be credited to the new filling.

COMPOSITE (WHITE) FILLINGS

If a composite restoration is the recommended treatment of choice, we will replace or repair it in the event of failure for a period of 3 years. If the tooth breaks within 3 years and requires a crown or onlay, we will credit the cost of the filling towards the crown or onlay if the patient presented for every 6 month recall appointment.

CROWNS, BRIDGES, INLAYS, ONLAYS AND PROCELAIN VENEERS

We will warranty these most comprehensive procedures for a full 5 years. If our crowns, porcelain veneers, or onlays need replacement, and the tooth is still savable and you do your part (see below) we will:

Replace any crown with a new crown of the same type

Replace any porcelain veneer with a new veneer

Replace any onlay with a new onlay

For the first three years at no charge

For the fourth year at 50% fee

For the fifth year at 80% fee

We cannot predict if and/or when gum or nerve treatments (such as a root canal) may be required. If needed, they would require additional fees. If the fracture is due to patient negligence (such as chewing on ice or very hard foods) or trauma, the warranty will be void.

This warranty does not cover accidents that cause damage to the teeth. All services must be performed in a Just Smiles office in order to be considered valid. You must visit our office for a minimum of 2 times (4 if we recommended due to gum condition) per calendar year for professional cleanings. If you move out of central Ohio, one professional cleaning & check-up in our office annually will be sufficient to maintain this agreement! You also must complete (in our office) all recommended treatment in the quadrant (quarter of the mouth the tooth in question is in) and adjunct procedures such as nightguards if recommended. Your path to optimal health requires a comprehensive approach to care.
